Rather than expand the necessary middle mile and backhaul, they use ITMP aka "traffic management" (http://www.wholesale.bell.ca/internetman.asp and http://www.dslreports.com/shownews/Bell-Backs-Off-Throttling-116665) and UBB (usage-based billing).
